By enrolling as a member of Kennedy Direct Primary Care, you agree to these Terms of Service. A formal member agreement will be provided at enrollment. These Terms constitute a binding agreement between you and Kennedy Direct Primary Care, operated by Eric Kennedy, MSN, APRN, FNP-C, and Lacy Kennedy, MSN, APRN, FNP-C.
Your monthly membership fee covers unlimited primary care visits, including:
Membership does not include: specialist care, hospitalizations, emergency services, imaging, surgery, or medications and laboratory services. Medications and labs are available to members at wholesale or discounted pricing and are billed separately at cost.
Kennedy Direct Primary Care is not health insurance and cannot be used as a substitute for health insurance coverage. Membership does not provide coverage for hospitalizations, emergency services, specialist referrals, imaging, surgery, or other services outside the scope of primary care. We strongly encourage all members to maintain health insurance or other coverage for these needs.
We strive to schedule members promptly, often the same or next day depending on clinical availability. Specific scheduling cannot be guaranteed and is subject to provider availability. Prompt scheduling is a priority of our practice model but is not guaranteed for all situations.
Either party may terminate membership with 30 days' written notice. Kennedy Direct Primary Care reserves the right to terminate membership for non-payment, violation of these Terms, or conduct that compromises the safety of staff or other patients.
Kennedy Direct Primary Care's liability for any claim arising from membership is limited to the amount of membership fees paid in the 3 months preceding the claim.
These Terms are governed by the laws of the State of Florida. Any disputes shall be resolved in Gadsden County, Florida.
